Signature Removal Information
Requirements
To remove your signature from the State Redistricting Petition please use this linked FORM.
In place of the form, you may mail (60 N. Main Street, P.O. Box 128, Coalville, UT 84017) or provide in person (60 N. Main Street, Coalville, UT 84017) to the Clerk the following information:
- The name or description of the petition from which you seek to remove your signature
- Your name
- Your physical residence address at which you live and are registered to vote
- Your signature (or if you have a qualifying disability under the ADA, you may affix "AV" as your signature and provide a phone number or email)
- The date signed
- To increase the likelihood of your signature being identified and removed, your statement may include your birth date or age
Electronic or emailed statements cannot be accepted.
DEADLINE
Your statement requesting your signature be removed from the petition must be submitted to the County Clerk's office either 1) within 30 days of when you signed the removal form, OR 2) within 45 days after your name appeared on the Utah Lieutenant Governor's website, whichever date is earlier applies to you. The statement must be submitted by mail or hand-delivered to the Clerk's Office (60 N. Main Street, P.O. Box 128, Coalville, UT 84017) by 5 pm on the applicable date. No electronic or emailed statements may be submitted.
